Person-Centered ApproachOur tailored-day service, supported employment program and community integration training utilize a person-centered approach to provide services to individual consumers. This means our staff are trained to assess each consumer's strengths and preferences, balancing what are "important to" and what are "important for" the consumer in order to design an individualized service plan to achieve their desired outcomes. We implement a holistic approach that includes ongoing communication and collaboration with a consumer's family and circle of support.

Person-Centered Planning ServicesHelping Hands East Bay is vendored with the Regional Center of the East Bay to provide Person-Centered Planning services to consumers at no cost to their families. Person-Centered Planning is a helpful process to discover a consumer's strengths, desires, dreams and long-term goals for the future. A Person-Centered Plan is a helpful step to prepare for transition to the Self-Determination Program.
Helping Hands East Bay's bilingual staff focuses on serving Cantonese and Vietnamese-speaking adult consumers in our person-centered planning services. For more information about this service, please contact Esther Chow, Founder & Executive Director at [email protected] |
Self-Determination ProgramBeginning on July 1, 2021, the Self-Determination Program is a new model of service delivery available to all Regional Center consumers who would like to choose this option. Self-determination gives you more freedom and control to choose the types of services that best support you. Unlike the "Traditional Model," Self-Determination allows you to choose services and support staff that do not have contracts with the Regional Center system. To learn more, visit www.rceb.org
Under a grant from the Department of Developmental Services, Helping Hands East Bay provides free coaching to underserved Cantonese, Vietnamese and Filipino adults and their families to assist them if they wish to transition to the Self-Determination Program. |
